To Make a Suggestion

You can submit a suggestion one of the following ways.

1. Fill out a form at these locations:

Seattle:  Near the coffee stands in Building 100 and Building 1
American Lake: Canteen, Building 2, 61 and 85.
Outpatient Clinics: Forms will soon be added to these locations.

2. Print out the pdf form (text version) and send it via mail to:

3. Print out pdf form (text version)and drop it off at one of the locations listed above.
